Affidavit

With the help of a notary, create an affidavit for a name change. You'll have to explain why you want to change your name. For example, in some cultures, a woman is given a new name after marriage, which she may choose to change officially, or which someone else may wish to change due to the fact that it is overly long or difficult to say.

Publish in newspaper

The name change must then be publicized in at least two local publications. One should be published in a National daily, while the other should be published in a regional publication. This is to introduce your new name to the general public. Always have a copy of the newspaper with you.

Gazette notification

You should have it published in the official gazette of the state where you live after it's been published. To do so, go to the gazette office and fill out a deed changing name form. Make several copies of everything. Send the copies to the office to be published as a gazette notice in the government press.

Documents Required for Name change in Education Certificates

• A letter to the secretary declaring you want to alter your name and requesting permission to do so
• The deed changing name form
• At least two copies of the local newspaper in which your name was published
• Attested a copy of the affidavit
• A photocopy of your present ID (PAN card, voter ID card, passport, etc)
